All dog license applications MUST include a rabies certificate that is valid for twelve months from your payment date. One year rabies vaccinations may be exempt. (see #5)
All required certificates/documents must be photocopies that may not be returned to you.
Complete one application for each dog (must be four months or older). Applications are only for dogs maintained in the City of Westminster that have not been licensed here before.
The license fee for a dog is $65.00 unless you are eligible for one of the following:
$15.00- Spayed/Neutered Fee: You must include a copy of a spay/neuter certificate or a certificate of sterility. A receipt from your veterinarian or a rabies certificate is not proof of spay/neuter, unless it states on the receipt it is a spay/neuter certificate and it is signed by the veterinarian.
$6.50- Senior Citizen Fee: You must be 60 or older and include a copy of a document showing proof of age, and the dog must be spayed or neutered. You can get the senior discount on the first spayed or neutered dog (send a copy of the spay/neuter certificate), and one such discount is allowed per senior citizen. Any additional dogs are at regular or spay/neuter fees.

Failure to submit required documentation and/or correct fee prior to the due date will result in a $25.00 penalty in addition to the license fee.
