IMPORTANT – READ BEFORE EXECUTING AND FILING CLAIM AGAINST THE CITY FORM

  1. Claims for death, injury to person, or to personal property must be filed no later than six months after the occurrence (Gov. Code, Sec. 911.2).
  2. Claims for damages to real property must be filed not later than one year after the occurrence (Gov. Code, Sec. 911.2). This form is for the convenience of those desiring to present claims against the city. Claimant is advised to consult his/her private attorney if he/she desires legal advice. No employee of the city may give legal advice to any claimant relating to private claims.
